Records identified in the indexes may be viewed and information may be copied from them, however the supplied copy of the vital record must be returned and is not permitted to leave the building. Include with your request as much as you can of the following: full name of decedent, date of death, city or county of death, your relationship to the individual, your name and address and a daytime telephone number.

As with birth records you may, however, perform searches in the indexes at the Ohio Department of Health, Office of Vital Statistics, and view and copy information from the death records themselves. Comments: Copies of marriage records are not available from the State Health Department.
